The Ride Documentation document is for rider information. If a co-driver should also be included on the certificate, then also state this on this document.
Clearly state what type of certification it is. If you experienced a special event during the ride, you can also mention it in this form.
There is room available on the form for name and your motorcycle, to be written on the certificate.
